Description

The individual will plan, coordinate and implement client programs, with guidance and oversight from the Assistant Director, and based on needs of program at the time. Assisting clients with personal care needs is a component of the position. Additionally, this individual will coordinate participation of volunteers, a regular component of the program. 

Requirements

•  Plan, coordinate, implement and evaluate a diverse set of therapeutic group activities. This includes monitoring and ordering/shopping for activity supplies as needed.
• Provide direct client care, assist with personal care needs, meal service, activity supervision and assistance. This includes transfers, orientation, ambulation and bathing as needed.
• Serve as a care coordinator for a designated caseload.  This includes meeting charting requirements and communication with clients and families as needed.
•  Conduct assessments of client progress to evaluate leisure, quality of life and cognitive functioning.
• Screen, orient, and onboard program volunteers. Evaluate volunteer participation and ensure it meets expectations for both the volunteer and the program and positively enhances client experiences.
